Farmington Health Services, also known as Trinity, has it's origins in Farmington MN since the 60's. We've grown considerably since its opening. Trinity is a senior campus that provides services to those that need Long Term Care within a Skilled Nursing Facility. We also provide short term rehabilitation through Big Stone Therapies. BST provides both internal and outpatient rehab services, such as physical, occupational and speech therapy. Trinity Terrace is our Independent or Assisted Living Facility. Newly opened in 2012 is Trinity Suites, our specialized Memory Care Home.
